Hospital Partners:

FGC partners with several Illinois hospitals to provide a variety of services including the following:

Hospital-based Assessment/Linkage Services for Patients with OUD (Warm Hand Off Services). The opioid crisis in Illinois has resulted in an increase in opioid-related emergency department and hospital visits across the state. To address this issue, FGC has received funding to provide OUD screening, recovery coaching, and “warm hand-off” services to persons presenting in hospital emergency departments. FGC recruits and maintain a team of recovery coaches and counselors who are co-located in 7 Illinois hospitals in high-need areas and act in a facilitation role to triage persons with indicated OUD to local MAT providers. FGC staff work with the hospital’s medical team to create a continuing care plan for individuals who present in the ED with heroin or other illicit opioid use. Participating patients can expect to receive substance use disorder education, a clinical assessment, a continuing care plan, and a firm community referrals following discharge/release from the hospital

SBIRT

FGC provides Screening, Brief Intervention, and Referral to Treatment (SBIRT) services to adults. SBIRT Screening and Brief Intervention (BI) services are provided by FGC and the hospital’s medical staff who complete BI training.

E-COVID

FGC provides E-COVID related services in which all hospital patients are screened (SBIRT) to increase engagement and access to SUD and recovery support services for persons with mental health and substance use disorders (not just opioid use disorders but also alcohol use disorders) who have been impacted by the COVID-19 pandemic crisis.
